The Doctor would be all like “Wizards! Fantastic” and Hermione would do her sassy eye roll and show him what’s up. The Doctor would immediately recognize her as a time traveler and she would explain how the T.A.R.D.I.S works both in terms of science and magic. She’s show him the bag she put a spell on and the Doctor would smile because he already knew she was the most brilliant witch of her time (and several others). Hermione and the Doctor would have total brain/friend crushes on each other (nothing romantic or sexual about it) and get along wonderfully. Ron would be jealous for a bit but then he’d end up liking the Doctor. Harry would take a while to come around because the Doctor wouldn’t let him go back to save his parents or Sirius.
Let’s be honest, if this happened it would be all about the Doctor and Hermione.
