Look at this for a second. Think about it. What age to you get spoon fed like that? Before lasting memories, certainly. That means Dean won’t remember Mary doing it for him. John obviously didn’t do it. Dean wouldn’t watch tv shows that are like that. So where would he pick this up?
I can only see one option.
Bobby. When John was hunting, Bobby would do it for Baby Sam whilst Dean watched. Sam would be up in his little high chair that Bobby made himself, with Dean’s help of course, and Bobby would feed him because John wasn’t there.
And when Bobby wasn’t there, Dean would do it for Sam. Dean always emulated his father figure and in caring for Sam, that was Bobby. That’s why he’s doing it now. Sam is in a very childlike frame of vulnerability which causes Dean to look after him like a kid and all that comes back to him. Sam’s a little confused because he has no idea why Dean is treating him like a kid.
And look at Dean. He throws it out like a joke but he knows Sammy doesn’t remember him caring for him as a kid. He knows that he’s weak and he won’t accept care, but Dean has always been caring for him and he doesn’t remember.
