Patient continues to exhibit signs of Post-Traumatic Stress Disorder. Wakes up several times each night and remains mute until he falls back asleep. Severe psychological responses when confronted with idea of leaving ground floor – has frequently needed to be restrained to prevent patient from harming attendants. Psychosomatic limp has returned. Memo from Doctor Thompson that patient underwent a panic attack when asked about Sherlock Holmes, deceased friend of subject, during last appointment. Possible harm to self – patient refuses to eat or is unable to hold down food. Possible harm to others. Change in medication recommended. Isolation recommended. Further observation is required.
